Skilled Nursing

Sometimes patients need additional nursing care before returning to their home or care facility. The Skilled Nursing/ Subacute Unit at San Gabriel Valley Medical Center provides care for patients who have completed the acute care phase of their hospital stay, but need further medical assistance and rehabilitative care. Our team of physicians, nurses, dietitians, and therapists work closely with patients and their families to coordinate all aspects of the care provided.

Subacute Care

The Subacute Unit is a designated area inside SGVMC that cares for patients who require complex medical technology and support to sustain life. Our goal is to help each patient achieve the best quality of life and the highest level of functioning possible.

Providing comprehensive inpatient care for the medically complex patient with an acute illness, injury, or exacerbation of a chronic disease, the care in this unit is given immediately after or instead of acute hospitalization. Patients requiring long term treatment for respiratory conditions are cared for in this specialized area of the hospital.

Because San Gabriel Valley Medical center is a full-services hospital with a skilled nursing facility, we provide a multi-disciplinary approach to care. The full range of health care services and programs we provide contributes to the physical, psychological, social, and spiritual well being of our patients and their family members. Surveys by the Joint Commission on Accreditation of Healthcare Organization, the Department of Health Services and the California Medical Association Institute of Medical Quality consistently give high scores to our services and programs.

What Type of Patient Should Use the Subacute Care Services?

The Subacute Unit specializes in caring for patients who have:

Multiple trauma, including head and/or spinal cord injury
Degenerative diseases such as multiple sclerosis, muscular dystrophy, or amyotrophic lateral sclerosis, cerebral vascular accident and other neurological conditions with respiratory failure
Respiratory failure

These patients may require:

Respiratory care, including ventilator support
Tracheostomy care and nutrition with feeding tubes
Pre-and post-rehabilitation, including occupational therapy, physical therapy, sensory stimulation, speech therapy
Wound care

The Multidisciplinary Subacute Care Team

Each patient has a multi-disciplinary care team including: a Social Worker, Activities Director, Clinical Director(Registered Nurse), Medical Director (Subacute Specialty Physician), Respiratory Therapist, Physical Therapist, Occupational Therapist, Speech Therapist, Pharmacist, and Nutritionist. Every patient in the Subacute Unit is approached with compassion and dignity. Families are considered an integral part of the health care team and are kept up-to-date through regular biweekly or monthly meetings.
